LETTERS OF TEMPORARY GUARDIANSHIP
OR CONSERVATORSHIP (GC-150)


NOTE:
You will need a copy of the English-language Judicial Council form GC-150 to follow these instructions. Also:



Filling out the Caption on your form

Attorney or Party without Attorney” - if you do not have an attorney, fill in your name, address, and telephone number.

If you would like the court to communicate with you by fax or email, you may give them that contact information, but you do not have to. 

Attorney For (Name)” – if you do not have an attorney, write “self represented.”

SUPERIOR COURT OF CALIFORNIA, COUNTY OF” – write the name of the County where you are filing your case. Click here for location of courts.
Write the name of the court where the hearing will be held.
Check the box “Guardianship” and either “Person” or “Estate” - write in the full name of the minor (child) Check the box “Minor”.
Check the box “Guardian”.
For Court Use Only” – Leave blank.
Case Number” – Write in the case number.

You must get the Temporary Letters after the court hearing or the Guardianship Order will not be effective. You can get these letters from the court clerk.

Print the temporary guardian’s  name and check the appropriate boxes (“guardian” and “person” OR “estate”). Write the child’s name.

Add up all the pages that are attached to this form, and write that number here.

LEAVE THE NEXT AREA BLANK. The court clerk will stamp and sign this form here.



AFFIRMATION


Check the box for “guardian”. Write the date and the place (city and state) where you are now, then sign the form.
